complete warfarin_v5 possible_actions

WarfarinModel_v5.possible_actions() method just works for dose only. I should implement interval only and both too. This requires specialized normalizer that is one-to-one, does not change the size of the normalized output for different intervals. Some ideas:

• normalized discounted sum of doses could capture both dose and interval.
